Over Usage Bandwidth Notification

To get to the Over Usage Bandwidth Notification form:

1. Log into the EPHost control panel (http://cp.ephost.com)
2. Click the 'Account Settings' icon
3. Click the 'Over Usage Bandwidth Notification' icon
